EXTRA HEAVY TRUCKS TRACTORS OVER 45,000 Lbs. GCW
Guide - Eligibility and Requirements:
This program is intended for Truck-Tractors over 45,000 lbs. GCW. It does not matter if the Tractor is single rear axle or dual rear axle.
If the GCW is over 45,000 lbs, it is classified under this category.
Trailers - all types of trailer with a load capacity over 2,000 lbs.
Semi-Trailer - Trailer with a load capacity over 2,000 lbs with wheels at rear only, equipped with a fifth wheel coupling device and hauled behind tractors.

Binding
A picture of the vehicle(s) is required to obtain a Binder.
Application
Use commercial auto application. Extra-Heavy Truck - Tractor
For binding signed Application by the Insured and the Agent is Mandatory.
The Extra Heavy Truck Tractor application is now in an online quote-style format. If you click on Commercial Auto-Extra Heavy Truck-Tractor via the e-apps or PDF of the applications tab, the user will be directed to the familiar ‘create a new quote’ window.
The process is very similar to the existing commercial auto online rater, HOWEVER, agents will only be able to submit to company for underwriting review.
The ‘application’ can be saved for future changes. Also, the users can upload pictures and loss runs along with their submission. As in the existing e-Apps system, the users will get a copy in PDF format of the application they submitted to us via email.

Maximum Liability Limits $1,000,000 CSL BI & PD.

Underwriting Criteria
- Multiple Named Insureds - UNACCEPTABLE
- No physical damage coverage for vehicles over 15 years old.
- Physical damage will only be written in conjunction with liability coverage.
- We will not accept any driver/operator whose drivers license has been suspended or revoked with the last three years or who has more than 10 points within the past three years.

627.7415. Commercial motor vehicle; additional liability insurance coverage
Commercial motor vehicles, operated upon the roads and highways of the State of Florida shall be insured with the following minimum levels of combined
Bodily Liability Insurance and Property Damage Liability Insurance in addition to any other insurance requirements:
Three hundred thousand dollars per occurrence for a commercial motor vehicle with a gross vehicle weight of 44,000 pounds or more

- Vehicles used to transport animals
- Flammable liquid haulers
- Dump trucks used for sand and gravel hauling
- Mail carries or message services
- Concrete trucks
- Vehicles used to haul heavy machinery or equipment
- Logging vehicles
- Long haul truck men
- Car carriers
Prohibited Classes:
Any Risk Hauling, Transporting, Storing, or Handling the following:
- Sand and Gravel Haulers
- Chemicals
- Explosives
- Flammable Materials
- Oil
- Gas
- Hazardous Waste
- Toxic Substance
- Ammunitions or Fire Arms
- House Movers
- Mobile Home Transport
- Risks that haul garbage, debris, or refuse to a dump
- Waste and Recycle Material
